New inspection forms
Information collection
Under the new Childminding and Day Care regulations and National
Minimum Standards in Wales, the Care and Social Services
Inspectorate Wales (CSSIW) has made some changes to the way they
collect information from childminders as part of the inspection
process.
The Self Assessment Form has been replaced with two new
forms:
- Annual Data Collection (ADC)
form – this asks for information about service users, staff and the
setting.
- Self Assessment of Service
(SAS) form – this asks for information based on the National
Minimum Standards.
These forms and guidance on how to complete
